Zunächst erfolgt mit dem an seinem vorderen Ende
diamantierten
Spiralbohrer
eine
unterdimensionierte
Bohrung
in der Achse und im Zentrum
der Zahnwurzel
oder des noch
vorhandenen Zahnwurzelfragments.
Diese
sollte eine Tiefe von ca. 7 mm im festen
Hartgewebe
haben, eine tiefere
Bohrung
ist nicht notwendig.
Idealerweise
erfolgt der Einsatz
mit
Wasserkühlung.
Zur
besseren Entfernung
der Bohrspäne
sollte der
Spiralbohrer etwas
ein- und auswärts bewegt werden.
Initially, a diamond coated twist drill or standard twist drill is used to
perform an under-
dimensioned drilling operation on the axis and the centre
of the root or the
remaining root
fragment. The drill should reach a depth of

unnecessary. Ideally, this action should be carried using a water cooling

Der Extraktor wird mit der zahnschonenden Teflon®-fläche auf den benachbarten
Zahnkronen positioniert.
Dabei
wird die Öffnung des runden, drehbaren Abstützteils
nach
vestibulär gerichtet, wobei
eine freie Sicht auf
die Extraktionsschraube
gewährleistet sein sollte. Dann
wird das Zugseil an der Extraktionsschraube eingehakt, über die
Umlenkrolle geführt und an dem Haken des
Extraktionsschlittens
eingehängt. Schraube und Seil
sollten hierbei die gleiche axiale Richtung aufweisen.
The extractor will be positioned on top of the adjoing teeth, which are protected by the Teflon® bite plate. The opening of the round, rotatable bite plate will be
directed
towards the vestibule, which should ensure
an unobstructed view of the extraction screw.
Subsequently the traction string will be secured into
the extraction screw, guided
over the return pulley
and hung onto the hook of the extraction slide while the screw and the string
should be pointing in the same axial direction.
Nach korrekter Positionierung des Extraktors erfolgt die Extraktion durch
kontrolliertes und langsames Drehen der Handschraube. Bei langen und kräftigen
Wurzeln sollten
die Parodontalfasern in Intervallen mit submaximaler
Seilzukraft vorgespannt werden. Um eine optimale, vertikale Kraftübertragung zu garantieren,
muss eine rechtwinklige
Ebene zur Auflage des Extraktors
vorliegen. Die neu entwickelte Quadrantenstütze
bietet diese rechtwinklige Auflage-Ebene. Ebenso kann
eine schlechte und unebene
Auflagefläche einfach korrigiert
werden. Notwendig wird der Einsatz dieser Quadrantenstütze
in erster Linie bei der Extraktion im Unterkiefer
sowie bei palatinalen Wurzeln
im Oberkiefer.
Once the extractor has been correctly positioned, extraction takes place by means of slow, controlled turning of the handscrew. If the roots are long and
powerful, the
periodontal fibres should be pretensioned at intervals using periotomes. The best possible vertical transmission of power requires
the existence of a right-angled plane to
rest the
extractor on. The newly developed quadrant support
offers such a right-angled plane for positioning the
extractor. This quadrant allows for uneven or open
ended
situautions to be corrected. Primarily, the use
of a quadrant support is required for extractions in the
lower jaw area as well as for extracting palatal roots in the upper jaw.
