Bei fortgeschrittener horizontaler Resorptionsklasse
(wenn Bone-Spreading nicht
mehr möglich ist) ist eine
Anlagerungsplastik zur Kompensation erforderlich.
In cases of severe horizontal alveolar ridge resorption
(where even bone-
spreading techniques cannot be
deployed) horizontal augmentative grafts are
required for
compensation purposes.
Je nach notwendigem Implantatdurchmesser wird zunächst mit
dem
geeigneten Lagerfräser (Durchmesser beachten) in Zahnachse
am horizontal
resorbierten Alveolarfortsatz ein Knochenlager
vorgefräst. Der Fräser ist dabei
so beschaffen, dass
das
Empfängerlager gut angefrischt wird. Der blutende
Untergrund dient der besseren Einheilung.
Initially an ablative bur (ensure diameter) is chosen and utilized to

standardised cavity against the horizontally resorbed
alveolar ridge according
to the diameter of the subsequently
inserted implant.
The surgical technique
and the properties of the
ablative bone bur lead to a hyperaemic environment,
which
accelerates the wound healing in post transplantation.
Anschließend wird mit dem zum vorher eingesetzten Lagerfräser passenden Trepan z.B. aus der retromolaren Region des Unterkiefers ein durchmesserkongruenter
Knochenzylinder entnommen. Dieser kann bei Bedarf mit der Osteotomiescheibe oder dem Osteotomiediamanten vom Spenderknochen gelöst werden. Die Spenderregion
kann nach der Knochenentnahme mit Collagen aufgefüllt werden. Ein üblicher Nahtverschluss beendet den Eingriff.
With the appropriate trephine, corresponding to the previously used ablative bur, a bone cylinder congruent to the diameter of the recipient region is extracted e.g. from the
retro-molar or other selected donar site. If necessary, the osteotomy disk or diamond can be used to create a horizontal cut allowing the bone cylinder to be taken from the
donor site. The donor site may be filled with collagen or other grafting material. The intervention is terminated by a current gingival suture.
Der entnommene Knochenzylinder wird zur Kompensation des horizontalen
Knochendefizits in das Empfängerlager eingesetzt. Bei guter Klemmwirkung kann
eine zusätzliche
Fixierung entfallen.
The extracted bone cylinder is placed into the recipient bone site to compensate
the horizontal bone deficit. A satisfying press fit may render additional fixation
unnecessary.
Zeigt der Knochenzylinder im Lager noch Mobilität, so kann er mit einer
Fixationsschraube aus dem optimal auf Transfer-Contol abgestimmten
Screw System (Art.-No. BSR00, BSRPR) fixiert werden.
If a certain mobility of the autograft in the cavity remains, a fixation screw
from the Screw System (Art.-No. BSR00, BSRPR) which optimally
corresponds with Transfer-Control, may be implemented.
Bei fortgeschrittener vertikaler Knochenresorption ist eine Auflagerungsplastik zur Kompensation erforderlich. Dabei wird das Empfängerlager auf dem krestalen
Alveolarfortsatz statt mit den Lagerfräsern mit den entsprechenden Radfräsern vorbereitet. Auch hier ist auf den Einsatz der durchmesserkongruenten Trepane zu
a
chten.
In cases of severe vertical alveolar ridge resorption, vertical augmentative grafts are required for compensation. In this case the appropriate wheel-cutter creates a
congruent cavity in the crestal alveolar ridge. One should take note of the correlating
trephines to be used. Prepare an autogenous bone graft in the same manner as
described for horizontal defects.
