Als erstes wird mit dem Pilotbohrer eine Bohrung
mit mäßigem Druck
und unter Wasserkühlung
parallel zur Wurzellängsachse und bis zum
E
nde der
Pilotbohrerschneide durchgeführt.
First, use moderate pressure and water-cooling todrill a pilot hole parallel
to the longitudinal axis of the
root until you reach the end of the pilot drill
blade.
Den passenden Wurzelanker mit aufgesetzter Eindrehhilfe und Ratsche langsam bis
zum vollständigen Verschwinden des Gewindes in der
Pilotbohrung versenken. Danach
werden Ratsche und Eindrehhilfe
abgenommen.
Slowly insert a suitable root anchor with attached insertion device and
ratchet until the
thread disappears completely into the pilot borehole. Afterwards, remove ratchet and
insertion device.
Die Auflageplatte mit den unterschiedlich hohen
Abstandshaltern so auf
den Nachbarzähnen ausrichten,
dass die Auflageplatte parallel zur
Okklusionsebene
verläuft und der Wurzelanker zugleich mittig durch ihre
Öffnung ragt. Liegt der zu entfernende Wurzelrest nicht
in einer Linie mit
den Nachbarzähnen, etwa bei der
Extraktion eines Eckzahnes, so werden
die
Abstandshalter über Eck in die Auflageplatte
eingesetzt.
Align the support plate with the spacers of various
heights with the
neighbouring teeth in such a way that
the support plate runs parallel to
he occlusion level
with the extraction anchor protruding centrally through
its opening. If the retained root that is to be removed is
not in line with
the neighbouring teeth i.e. if you are
extracting a canine tooth, then the
spacers have to be
removed and placed in an angle.
Bei Endständigkeit des Wurzelrestes wird die Auflageplatte mit dem
flachen
Abstandshalter auf dem Nachbarzahn und mit dem hohen
Abstandshalter auf dem
Kieferkamm positioniert. Als nächstes wird
die Extraktionsschraube, der im Bedarfsfall
eine in der Länge
passende Distanzhülse aufgesetzt wird, durch die Öffnung der
A
uflageplatte gesteckt und per Hand auf den Wurzelanker
aufgeschraubt.
If the retained root is terminal, the support plate is placed on the
neighbouring tooth
w
ith the short spacer and on the mandibular ridge
with the tall spacer. Next, insert the
bore-aid for root anchor, which,
in some cases, may need to be covered with a
distance sleeve of
corresponding length, through the opening of the support plate and
screw manually onto the root anchor.
Jetzt wird die Ratsche auf die Extraktionsschraube aufgesetzt. Mit einer Hand wird die Auflageplatte gegen Verrutschen gesichert,
mit der anderen die Ratsche langsam im
Uhrzeigersinn gedreht. Bei festsitzenden Wurzelresten jeweils nach einer Drehung um ca.
30˚ bis 60˚ für 5 bis 10 Sekunden pausieren, um den Scharpey´schen Fasern des
Desmodonts Zeit zum Zerreißen zu geben.
Erst dann weiter drehen. Die Luxation kommt schließlich zum Ende, wenn nach dem anfänglichen Andrehen gegen einen
gewissen
Widerstand, dieser plötzlich nachlässt und der Wurzelrest in toto zum Vorschein kommt.
Now place the ratchet onto the boring tool. Use one hand to prevent the support plate from slipping and use the other to slowly
rotate the ratchet clockwise.In the case of
trapped roots, pause for 5 to 10 seconds after having turned the ratchet by around 30
to 60 degrees in order to give the Scharpey´s fibres of the periodontal ligament time
to tear. Only then continue rotating. The
luxation finally ends as soon as the resistance that you encounter after the initial turning suddenly recedes and the entire retained
root appears before you.
